Foreign Travel Insurance Program
All faculty, staff, and students traveling internationally on University business are required to be enrolled in the Â鶹´«Ã½ÉçÇøÈë¿Ú Risk Management Authority (CSURMA) Foreign Travel Insurance Program (FTIP) prior to departure. All international travel must be reported to Risk Management to ensure coverage, as the insurance is not automatically applied.
Summary of Coverage
FTIP covers accident & sickness (including medical coverage), general liability, excess auto liability, foreign workers' compensation, and executive assistance (including legal assistance, embassy/consular contacts and assistance in replacing lost documents).
Request Foreign Travel Insurance
To request foreign travel insurance, send an email with a copy of the traveler’s completed Travel Authorization form to Risk Management at RiskManagement@csueastbay.edu. Insurance must be requested at least 45 days in advance of travel.
Travel to High Hazard countries must comply with the . Travelers are advised to check the and the System Wide Risk Management High Hazard & War Country List for the most current high hazard international travel warning information. Both lists are subject to change.
Faculty and Staff- Email Risk Management a signed copy of the traveler’s Travel Authorization Form or CC Risk Management on the form through Adobe Sign.
- Complete Foreign Travel Insurance Request Form. The link to the form will be emailed by Risk Management after the Travel Authorization Form has been received.
- Please note that premiums for each traveler will be recharged to the appropriate department at the end of the quarter.
- It is recommended that all employees traveling internationally sign up for the State Department's Safe Traveler Enrollment Program (STEP). This system provides information to travelers should conditions in the foreign country change and can assist in an emergency. The on-line enrollment site is at .
Students
- Email Risk Management a signed copy of the traveler’s Travel Authorization Form or CC Risk Management on the form through Adobe Sign.
- Email Risk Management a signed copy of the Waiver of Liability. Only wet signatures are accepted, as electronic signatures are not approved.
- Complete Foreign Travel Insurance Request Form. The link to the form will be emailed by Risk Management after the Travel Authorization Form has been received.
- If the traveler is participating in an International Student Exchange Program (ISEP) program, then email a copy of the traveler’s ISEP insurance documentation to Risk Management.
- The traveler will be enrolled in CSURMA Foreign Travel “Overlay” Insurance.
- This coverage is intended to supplement, not replace, the insurance required by the Study Abroad vendor who, under a written agreement with CSU, provides educational opportunities for CSU students to study abroad. Overlay insurance will act as an extension to the ISEP policy and will only provide General Liability insurance, but does not include Travel Assistance, Accident & Health or any of its associated benefits including but not limited to: Medical Expense, Emergency Medical, Emergency Medical Evacuation, Repatriation of Remains, Emergency Reunion, Home Country Extension, Security Evacuation, Trip Cancellation, Trip Interruption, Accidental Death & Dismemberment, Coma, Felonious Assault, Hijacking and Air Piracy, etc. Therefore, there will not be a duplicate in the insurance coverages.
